Re: Safety Alert

Hi All,

Thanks all, and keep those reports coming.

See direct pics attached of this specimen. The weld quality is such that I wonder if a previous repair was accomplished, am reviewing the logs again. Also we found interference between the LH assy support bracket and RH tube that may have contributed to the leverage. The parts were generally clean so I think we can rule out corrosion as the cause.

Re: Safety Alert

Hi All,

Smoking gun!

Our incredible wizard of Commander histories, Judi, found the smoking gun! An incident that found its way into one of the aircraft databases, but not the logs, 9 months after the airplane got its COA. Same exact failure, clearly explaining the repair-looking weld.

Keep checking your planes. This information does not explain the original failure, when she was a newbie airplane.

From the above seems you have found plausible reason. In order for us to check ours - can you explain the interference that you found?

Re: Safety Alert

Hi Frank,

The LH and RH pedal support tubes are parallel with only approx 1/8" between them. On the specimen airplane, this was not enough to provide clearance for the tube support brackets to fit in between the tubes (114 IPC Fig 2-41, Callout 40). The steel tube cut the aluminum bracket to create clearance. This was probably not a significant contributing factor in causing the crank welds to crack, but stays on the list while we figure this thing out.

Re: Safety Alert

114BL looks good...

Re: Safety Alert

Had GBEPY's rudder pedals and linkage disassembled, cleaned, stained and carefully checked at annual this week and no cracks or signs of fatigue found.

On another happy note the elevator survived its 4th disassembly and again no cracks found.
