The status quo is being challenged and the training world is not equipped to address it and the manufacturers (particularly engines) would rather not address it if they don't have to. Having said that, proper LOP operations is best done with a well tuned fuel injected engine (balanced injectors)...

Re: What would you do? A certain amount of metal is allowed. there is a Lycoming service bulletin that specifies and amount of metal and what actions are required. I think it is: Service Instruction No. 1492D. You may be surprise how much metal can be found in the engine and still good to fly...
